Our practice welcomes and appreciates all our patients and referral sources.  If you are a new patient, to expedite your visit, please click the following link to open the

Insurances.  Our office accepts most insurance plans, including Cigna, Aetna, Regence, Premera, United Healthcare, First Choice and Kaiser Permanente (f.k.a. Group Health).  We verify for referral status, eligibility and deductibles and collect at the time of service.  Please see the Scheduling FAQs for more information.

Scheduling appointments.  Please call 206-682-8741 to schedule an appointment.  If we do not answer right away, we are likely on the other line or working with a patient.  Please leave a message, email us at or fill out our contact form, and include your phone number.  Our staff will get back with you promptly.

We strongly recommend that you familiarize yourself with these tips to make your experience with the practice as smooth as possible.  If you need to change your appointment, please do so at your earliest possibility to allow for other patients to be seen.  There is a fee for a no show, no call.
